DESERT DIVA POLICIES, TERMS & CONDITIONS PERMANENT MAKEUP RESTRICTIONS Service is strictly prohibited under the following conditions: Pregnant or nursing Under 18 years of age Currently undergoing chemotherapy, or with unmanaged diabetes, epilepsy, MS, or autoimmune disease (written physician approval is mandatory and must be emailed at booking to desertdivapmu@icloud.com) Use of Accutane or equivalent within 12 months, Retin-A within 4 weeks, or any laser/chemical treatment on the targeted area within the past 4 weeks If you are unsure whether a condition, medication, or treatment qualifies, contact Desert Diva prior to booking. Failure to disclose may result in denied service with full charges applied. For first-time PMU clients with prior procedures: Clear, unfiltered, makeup-free images in natural lighting must be submitted to @DesertDivaBeauty on Instagram via DM for approval before booking. Approval is mandatory. If previous work is not disclosed in advance or medical restrictions are ignored without valid physician approval, your deposit will be forfeited and full service will be charged. TERMS & CONDITIONS APPOINTMENT POLICY All bookings require a non-refundable deposit charged immediately. All payments, including deposits and prepayments, are non-refundable—no exceptions. No guests, children, or pets permitted in the treatment area. LATE & RESCHEDULE POLICY Changes must be made by calling/texting 480-817-2495. Minimum 7-day notice required to avoid a 50% late cancellation fee. 72 hours or less notice is considered a “no show” and is fully billable. Arrival 10 minutes late or more results in automatic cancellation and full charge. Medical cancellations require a doctor’s note submitted within the same week to info@desertdivabeauty.com for any consideration.  
 Results vary due to skin type, health, age, pre/post care, and lifestyle. Multiple sessions may be required. No refunds once treatment is rendered. Client honesty during consultation is non-negotiable; failure to disclose relevant info voids liability. “Unawareness” or non-disclosure of conditions or medications will not result in refund under any circumstances. Clients with past no-shows must prepay in full to rebook. All media captured during sessions may be used for marketing unless prior written opt-out is submitted.   We reserve the right to decline or discontinue service at any time, with or without cause. PMU-SPECIFIC TERMS Touch-ups are not included. Rates start at $150+
 Once final design is approved by the client, any desired changes post-treatment are not the liability of Desert Diva.
